Continuation of Coverage for Overage Disabled Dependent

An unmarried child of age 26 or older may continue as a dependent if the child meets the following conditions:

  • The child is incapable of self-support by reason of mental or physical disability.
  • The child is mainly dependent on the employee for support and maintenance.

REQUIRED DOCUMENTATION

Aetna requires the following documents to be completed and submitted. The employee instructions and important notes are all listed on page 1 of the documents.

  • For groups headquartered internationally, the forms below should be sent to TBS (globalbilling@tbsmga.com). TBS will then send the documents to our eligibility consultant for review. Please allow seven to 10 business days processing time after which a decision will be made (approval or denial). 
    • Continuation of Coverage Disabled Child Form - Global
    • Attending Physician Statement Form - Global
  • For WA and AK groups, the member should send the below forms directly to Aetna for review and approval. They can either be mailed or faxed to:

  • Kindly refer to the attached files of this article. Any follow up for the decision status should be done by the member through Aetna Member Services.
